2008 Registration Forms
are Available Here for You to Download and Print

Instructions for Completing the Registration Forms (Microsoft Word forms)

  • *Click on link and download and save form to your hard drive, DO NOT fill out the form in the web browser (Internet Explorer, Firefox, etc..,) You can not save it in the browser*
  • Fill in all applicable spaces (yellow boxes)
  • Use the tab key to advance to the next field
  • Save the completed form to your hard drive or floppy disk for future use
  • All fillable forms have changed this year. You cannot use old forms from last year.
  • Print as a two sided form (Emergency Information Form and Physical Evaluation), if possible
  • Circle the preferred telephone number of all emergency contacts on the printed copies
  • Complete all required signatures and bring all copies to registration.

1.___ Emergency Information Form – 4 copies required for each student, please sign each copy
2.___ Student Enrollment Sheet – 1 form required for each student
3.___ BCS Emergency Early Dismissal Form – 2 copies required -please sign each copy
4.___ BCS AlertNow Emergency Call System – 1 copy required for each family

OTHER FORMS
5.___ Welcome Back Letter from PTSA Chairpersons
_____ PTSA Clearinghouse Back-to-School Receipt – 1 per family (Hardcopy in packet)
6.___ BCS PTSA Student Directory and Email Information – 1 form required per family
*****NOTE: This is a tan brown hardcopy form pre-filled already in your registration packet.
***** This form MUST be turned in for each family, even if all information is correct.

FORM FOR NEW STUDENTS ONLY (download, read the policy, and sign pg. 3)
7.___Technology Acceptable Use Policy -Signature Page – 1 copy for each new student

PHYSICAL FORM FOR STUDENTS PARTICIPATING IN ATHLETICS DURING THE ACADEMIC YEAR
8.___ BPS Preparticipation Physical Evaluation - 1 form required for each 6-8th grade student participating, or planning to participate, in interscholastic athletics.
